Pediatric Associates of Johns Creek, PC has compiled a list of pediatric specialists. A copy of this list is available at our office. You may request a recommendation from your child's doctor.
In accordance with some insurance policies, a referral may be required for payment of a specialist office visit. If your insurance plan requires that you attain a referral, contact our referral coordinator at 770-476-4020. Please allow three to five business days for the completion of your referral. If you do not know if a referral is required, please contact your insurance company.
In order to complete your referral, our office needs the following information after you have made the appointment with the medical specialist:
The specialist's first and last name The location of the specialist's office The appointment date The reason for the visit The specialist's phone and fax number
*Referrals are required for most Tricare, Blue Cross Blue Shield HMO/POS and Aetna HMO/POS plans.
*United Healthcare and Cigna insurance companies sometimes require a written prescription.
