We ask that all new patients arrive at least 15 minutes prior to your appointment time. There are several forms to complete in order to prepare your child's chart for his/her visit. Our front office staff will explain these additional forms to you at time of check in. In order to expedite this check in process, we have provided the patient registration forms for printing and completion prior to your arrival. If you need to request records from your former pediatrician, please print the incoming records release form below. We recommend sending this form to your former pediatrician several days before your scheduled appointment with our office.
If you are bringing your newborn for his/her first visit, please remember to bring the hospital discharge papers with you.
In the instance that you may need to request records for a specialist visit or if you are transferring from our practice to another, please print and complete the form below. There may be a fee for the retrieval and/or copying of your child's records. Please allow at least one week to transfer your child's records. Records located in off-site storage may require additional fees and time. Be certain to include a valid phone number on this transfer out form, for our staff will be contacting you upon receipt.
